Welcome, EPAC families! Please review the Fall 2026 Production Handbook and complete the three required forms before your student’s first rehearsal/audition on Thursday, August 27.

The Handbook Acknowledgment, Conflict Disclosure Form, and Medical & Emergency Authorization Form are fillable PDFs. Download each form to your device, complete and save it, then return it by email to info@elevatepac.org. You may also print the forms and bring them with you on August 27.

Please complete one set of forms for each participating student.
